Letter(s)of Medical Necessity Assessment & Documentation

Letters of Proof Request for evaluation ,Leave of Absence (LOA Disability LOA )consultations, assessment, and referral letter, are often requested and required from current or former clients for various reasons (employment,release,court, school,volunteer work etc)

After assessment and evaluation, we are happy to write a letter/fill out forms to Support you to serve as proof that you’ve spoken with your therapist  and for completion of therapy sessions coupled with a proper evaluation with accuracy based on professional assessment. 

In many cases this document is not required however when required in a situation where there needs to be proof of therapy sessions this is available. 

This document will serve as a legal note, and will require you to submit 2 forms of identification.

Photo ID / Drivers License, Passport, Social Security Card)

copy of referral request/Leave of absence LOA forms/summons /court/order with matching name required to ensure that in our sessions the necessary subject matter is or has been addressed during your sessions.
