After assessment and evaluation, we are happy to write a letter/fill out forms to Support you to serve as proof that you have spoken with your therapist and for completion of therapy sessions coupled with a proper evaluation with accuracy based on professional assessment.
Disability Leave of Absence for Mental Health Therapists

* No Insurance Company Involvement: We do not work directly with insurance companies.
* Fee Structure:
* Initial Letter: $147 (payable upfront)
* Weekly 45min therapy sessions $95
Documentation: $147per week (required to maintain leave)
* Return to Work Assessment: 1hr$147 (included in your last therapy session)
* Return to Work Paperwork: $147 (payable separately)
* Session Packages: Consider purchasing a "Disability Extension of Work" (DEW) session package for potential cost savings. Alternatively, you can pay for weekly sessions individually.
* Superbills: We will provide you with a superbill for each session, which you can submit to your insurance company for potential reimbursement.
Leave of Absence Process:
* Payment:
Pay the $147 initial letter fee to initiate the disability leave process.
* Weekly Documentation:
* Submit weekly documentation for your leave of absence.
* This documentation ensures your leave is maintained and may be necessary for any potential insurance claims.
* Therapy Sessions:
* Continue attending weekly therapy sessions during your leave.
* Consider the DEW session package for potential cost savings.
* Return to Work Assessment:
* The return to work assessment will be conducted during your last therapy session.
* Return to Work Paperwork:
* Pay the $147 fee for the return to work paperwork, which will be provided to you.
